HERE ARE SOME GUIDELINES ON WHEN TO KEEP YOUR CHILD HOME:
TEMPERATURE
Students MUST remain home if he/she has a fever over 100º. Doctors recommend that students be fever-free, without medication, for 24 hours before returning to school.
DIARRHEA
VOMITING
CONTAGIOUS DISEASES/CONDITION
Students must not have vomited for 24 hours prior to returning to school.
Students must remain home when he/she shows symptoms of a contagious disease such as strep throat, conjunctivitis (pink eye), flu, impetigo, ringworm, or if head lice is suspected. Please feel free to call the school nurse if you have any questions.
Thank you for your cooperation in following these health precautions to protect your child and other children from illnesses. We know it can be difficult for working parents to have a sick child at home.
SPECIAL HEALTH CONCERNS
If your child has a specific health concern, please notify the nurse (asthma, severe allergy, diabetes, etc.) so that appropriate measure/planning can take place specific to your child.****See forms to the left of the page
EMERGENCY CARDS
It is very important that you complete your child's yellow Emergency Card. Please keep the information on the card current so that the school staff can easily contact the appropriate authorized person if your child becomes ill or is hurt while in school. For the safety of all children, the child will be released only to those people listed on the emergency card.
PHYSICAL EXAMINATION & REQUIRED IMMUNIZATIONS
Every child entering seventh grade is required to have a physical examination by a doctor. The state also requires proof from your doctor that the child has received certain immunizations before entering school. These requirements are 5 DPT, 4 Polio, and 2 MMR (Measles, Mumps, Rubella), 3 Hepatitis, 2 Varicella vaccine or doctor’s documentation of chicken pox disease, 1 Tetanus Booster (Tdap) and 1 Meningococcal Vaccine (upon initial entry to seventh grade. Documentation with the full date - day, month, and year - is required for each immunization. ****See forms to the left of the page
MEDICATION POLICY
In school, medication can be administered only by the certified school nurse-teacher. Sometimes medications need to be given during school hours. Certain procedures must be followed to ensure the safety of all children. No medication will be given, including over-the-counter products, without parental and medical authorization for administration. Forms may be obtained from the school nurse or can be downloaded from this website.
If your child needs to be on medication in school, please follow these guidelines:
PRESCRIPTION ORDER - When you go to the doctor, please have the doctor write an order for school that it can be left with the school nurse. Your physician can fax an order for medication to Exeter-West Greenwich Junior High School Health Office Fax:401-397-2479
LABELED PRESCRIPTION CONTAINER - At the pharmacy, ask for a container for school with a complete prescription label on it.
PERMISSION SLIP - Sign a medical permission slip.***See form at the bottom of the page
PERSONAL ADULT DELIVERY - Parent/guardian MUST bring the medication in to school personally, do not send medications in with your child.
SPECIAL MEDICATION CONCERNS
In most instances students may carry their own EpiPens and inhalers as long as proper physician and parental documentation is forwarded to the health office. Please contact the school nurse for the medical forms or download the forms from this web site. ****See forms to the left of the page
SCOLIOSIS SCREENINGS
Required by state law. Scoliosis screenings are performed in the seventh and eighth grade in accordance with the Department of Health Regulations to determine if a problem or concern may exist. If there is a concern or potential problem with the scoliosis screening you will be notified by letter.
VISION SCREENING
Vision screenings are performed in the seventh grade in accordance with Department of Health Regulations. If the child fails twice, a letter informing the parent of the results is mailed home. Please follow up with an eye doctor and return the form to the school nurse.
DENTAL SCREENING
It is in the best interest of your child that a dental exam be completed each year. However, Department of Health Regulations only require the documentation of one dental exam on the secondary level.
A child's ability to learn, perform and behave can depend on how a child feels physically. We ask that sick children remain at home if their illness is, or may be, contagious and could cause illness in other children. We ask your cooperation by calling the school and informing us as soon as possible so we can detect similar illnesses in others sooner.
If your child is not feeling well and you send them to school, please inform the school nurse and provide a telephone number where you can be reached that day.
If your child becomes ill at school, you will be called to pick him/her up. If your child has special medical or health problems, please inform the school nurse.
